Code Monkeys LLC is the developer of the HIPAA FORMS Service.  This statement applies only to the HIPAA FORMS Service and should not be considered as a statement from your health care provider.

How We Collect Information About You: Code Monkeys LLC and its employees on behalf of their HIPAA FORMS Service subscribers collect encrypted data through the submission of website forms on the subscriber’s websites that is either required by law, necessary to process applications, necessary to provide health care services or other requests for assistance through our subscription customers.

What We Do Not Do With Your Information: Information submitted to us via web forms is held in strictest confidence.

We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about applicants or clients who apply for or actually receive our services that is considered patient confidential, is restricted by law, or has been specifically restricted by a patient/client in a signed HIPAA consent form.

How We Do Use Your Information: The HIPAA FORMS Service is simply a service to transmit and store encrypted web form data and is not viewed or used by Code Monkeys LLC in any way other than to transmit and store the encrypted information.  The encrypted information may only be decrypted and viewed by the HIPAA FORMS Service subscriber from a secure password protected SSL encrypted interface and only by staff that have been given the appropriate permissions.

How the HIPAA FORMS Service subscriber uses this information should be covered within a separate notice of information practices and privacy statement provided to you by the HIPAA FORMS Service subscriber.

Information We Do Not Collect: Code Monkeys LLC does not collect any additional information from a web form submission beyond what is on the form and no information entered into the web forms will be used for promotional or research purposes.

Notice of Limited Identifiable Data Collection: Limited information NOT deemed to be private protected health information but are considered “identifiable” data such as name, phone number and email address may be stored separately from the full encrypted form data with the HIPAA FORMS Service HIPAA Compliant data storage solution as a means for the HIPAA FORMS Service subscriber and their staff to find submitted forms by those criteria.
